This form, also known as the Physician’s Report for Community Care Facilities, is completed by a licensed physician. It ensures the facility can meet the resident’s care needs and confirms that the individual meets the community’s admission criteria.
What’s Included in the Form 602a?
The form contains:
- Identifying information, including next of kin.
- Medical conditions and prescribed medications.
- Ambulatory status (how mobile the individual is).
- Details about any impairments or assistive devices used.
- Mental health status and cognitive abilities.
- A description of their capacity for self-care.
A Tuberculosis (TB) Test
A TB test or chest X-ray is required to confirm the absence of active tuberculosis. This is a mandatory step to ensure the safety of all residents and staff in the community.
Admissions Paperwork
Each facility has its own set of admissions forms, which must be completed before the move. These documents typically include:
